Wirral Heart Support Service provides specialist support, assessment, lifestyle advice and education for cardiac patients, their families and carers.

Cardiac Rehabilitation

The aim of the Cardiac Rehabilitation service is to help people recover and resume a full life as quickly as possible following a heart attack, angioplasty or coronary by-pass and to help people who have heart conditions such as heart failure or angina.

Some people think that, because they were in and out of hospital so quickly, their heart attack was not a serious event. But a heart attack is a serious event and cardiac rehabilitation is very important for those people as well.

We offer several rehabilitation programmes that are tailored to suit the individual’s need.

The service offers:

• Initial contact within two weeks of discharge (five days of discharge if you have had surgery)
• Full initial assessment of your heart condition and lifestyle
• Six weeks of education and exercise tailored to your need
• Twelve weeks of structured exercise individually prescribed
• The opportunity to exercise in the community after your rehabilitation is finished.

 

Who we are:

We are a multidisciplinary team of highly trained and experienced nurses, exercise physiologists, doctors and psychologists who will support you in all aspects of your rehabilitation. We offer rehabilitation at St Catherine’s, Birkenhead or Victoria Central Health Centre, Wallasey.

Accessing the service:

We accept referrals from GPs, Practice Nurses and hospital staff.
Heart Services referral form (NOTE: Only Wirral health professionals are able to access this form).

Intermediate Cardiac Clinic

 

Wirral Intermediate Cardiac Clinic (WICC)

The Wirral Intermediate Cardiac Clinic (WICC) aims to see people who have been referred by their general practitioner (GP) or other health professional for diagnosis, investigation or management of a suspected cardiac condition.

People who have urgent or complex needs will continue to be seen at the hospital. The WICC is for those people who can be safely managed in the community at St Catherine’s, Birkenhead or Victoria Central Health Centre, Wallasey.

We offer:

• Clinical assessment at one of the above locations
• Echo cardiography (ECG) - ultrasound of the heart
• Resting ECG (heart tracing)
• Exercise ECG (on a treadmill or bike)
• 24 hour blood pressure monitoring
• 24 hour ECG
• Cardio memo event recording (patient operated heart tracing, usually for palpitations)
• 7 day event recording (heart tracing for patients with intermittent palpitations)
• Spirometry
• Fast track to Wirral University Teaching Hospital NHS Foundation Trust’s Arrowe Park Hospital or Liverpool Heart and Chest Hospital when necessary.

We accept referrals from GPs and Consultants.

Following referral you may be asked to contact the service directly to arrange a convenient appointment.

Heart Failure Programme

"Heart failure" is a term used when the heart becomes less efficient at pumping blood around the body, either while you are resting or active. The term "heart failure" is unfortunate because it implies that the heart has actually failed. However, most people have some control over their condition by making changes to their lifestyle and taking prescribed medication.

Our heart failure programme offers:

• Initial contact at the bedside if you have been admitted to hospital
• Follow up appointments with symptom and medication review
• Home visits if you are too unwell to attend clinic
• Referral onto other specialist services if required e.g. psychology, Liverpool Heart and Chest Hospital or Wirral University Teaching Hospital NHS Foundation Trust’s Arrowe Park Hospital
• Referral to the Living with Heart Failure rehabilitation programme if appropriate

Living with Heart Failure Rehab.

 

Living with Heart Failure Rehabilitation

The aim of the Living with Heart Failure Rehabilitation programme is to help people recover and resume a full a life as quickly as possible after a diagnosis of heart failure.

The term "heart failure" is unfortunate because it implies that the heart has actually failed. However, most people have some control over their condition by making changes to their lifestyle and taking prescribed medication.

The Living with Heart Failure Rehabilitation programme aims to provide guidance, education and support to help individuals in the management of their heart condition. We also aim to promote quality of life and psychological well being.

We offer:

• Full clinical initial assessment with a named rehabilitation officer
• Eight week programme of education on all aspects of living with heart failure
• Eight weeks of structured exercise specifically designed for patients with heart failure
• The opportunity to exercise in a more formal setting at the end of your rehabilitation (if appropriate)
